EOD v3.1 Review and Conversion

After EOD v3.1 has been deployed

1. AJCC ID [995]: there were changes to the V9 AJCC ID values to match the AJCC's new numbering scheme. Logic provided for those who CANNOT recalculate across their database.
   • If Date of Diagnosis Year is 2018 or later, for the AJCC ID [995] within each specified Schema ID [3800], the new value is provided:
     • If AJCC ID = 52 and Schema ID = 09520 (Cervix Uteri) and TNM Edition Number = 9 or blank, then set AJCC ID = 9001
     • If AJCC ID = 19 and Schema ID = 09190 (Appendix) and TNM Edition Number = 9 or blank, then set AJCC ID = 9002
     • If AJCC ID = 21 and Schema ID = 09210 (Anus) and TNM Edition Number = 9 or blank, then set AJCC ID = 9003
     • If AJCC ID = 72.1 and Schema ID = 09721 (Brain) and TNM Edition Number = 9 or blank, then set AJCC ID = 9004
     • If AJCC ID = 72.1 and Schema ID = 09722 (CNS Other) and TNM Edition Number = 9 or blank, then set AJCC ID = 9004
     • If AJCC ID = 72.1 and Schema ID = 09723 (Intracranial) and TNM Edition Number = 9 or blank, then set AJCC ID = 9004
     • If AJCC ID = 72.1 and Schema ID = 09724 (Medulloblastoma) and TNM Edition Number = 9 or blank, then set AJCC ID = 9004
     • If AJCC ID = 72.2 and Schema ID = 09724 (Medulloblastoma) and TNM Edition Number = 9 or blank, then set AJCC ID = 9005
   • OR Recalculate to derive the AJCC ID across the database 2018 and later
   • No other changes are necessary

2. In situ: there were changes to the calculation tables for the Derived EOD 2018 fields in approximately 41 schemas where AJCC does not define Tis. The Derived EOD 2018 N and Derived EOD 2018 M will both be set to 88 for such cases. Logic provided for those who CANNOT recalculate across their database.
   • If Date of Diagnosis Year is 2018 or later, if EOD Primary Tumor = 000 AND Derived EOD 2018 T = 88
   • If Derived EOD 2018 N is not 88, set Derived EOD 2018 N = 88
   • If Derived EOD 2018 M is not 88, set Derived EOD 2018 M = 88
   • OR Recalculate to the Derived EOD 2018 fields across the database for 2018 and later
   • No other changes are necessary

3. Grade: there were changes to the calculation tables for the Derived EOD 2018 Stage Group in 8 schemas where Grade is considered for stage group determination. The calculation no longer prioritizes by timing, but now uses the highest grade captured between Grade Clinical [3843] and Grade Pathological [3844]. Also, for Retroperitoneum and Soft Tissue Trunk and Extremities, the Grade value of ‘H’ was added to the Grade tables in v3.0 but had not been added to the Stage Group calculation tables – this has been corrected. If Derived EOD 2018 Stage Group is collected, it will need to be recalculated.
   • If Date of Diagnosis Year is 2018 or later, if Schema ID [3800] in 00190 (Appendix 8th), 09190 (Appendix V9), 00381 (Bone Appendicular Skeleton), 00480 (Breast), 00430 (GIST), 00580 (Prostate), 00440 (Retroperitoneum), 00410 (Soft Tissue Trunk and Extremities)
     • Recalculate to the Derived EOD 2018 fields across the database for 2018 and later
   • No other changes are necessary

4. Grade: Derived Summary Grade [1975] has been defined. This field will be calculated at the central registries for all cases diagnosed 2018 and later. The more severe value from Grade Clinical [3843] and Grade Pathological [3844] will be used. Breast is a special case because behavior affects the priority. If this field is required for your registry:
   • If Date of Diagnosis Year is 2018 or later
     • If Schema ID [3800] = 00480 (Breast), Set Derived Summary Grade to the highest value found in Grade Clinical and Grade Pathology where the priority order from BEST to WORST is (Table can be found at: https://staging.seer.cancer.gov/eod/table/3.1/combined_grade_breast_8723/)
       • For Behavior /2: H, M, L, 3, 2, 1, D, C, B, A, 9, blank
       • For Behavior /3: 3, 2, 1, H, M, L, D, C, B, A, 9, blank
     • Else (all other schema), Set Derived Summary Grade to the highest value found in Grade Clinical and Grade Pathology where the priority order from BEST to WORST is (Table can be found at: https://staging.seer.cancer.gov/eod/table/3.1/combined_grade_56638/)
       • S, 5, 4, 3, 2, 1, E, D, C, B, A, H, M, L, 9, blank
   • No other changes are necessary

5. SEER Site Specific Factor 1 [3700]: the field is changing from a 1 digit fields to a 2 digit field. There is a conversion between the old values and the new.
   • For the Schema ID [3800], the conversion between the old and new values of SEER Site Specific Factor 1 is provided:
   • If Date of Diagnosis Year is prior to 2018, set SEER Site Specific Factor 1 to blank.
   • Else If Date of Diagnosis Year is 2018 or later

• General Changes
  • LVI: LVI was removed from all schemas. The values shown had been limited to those applicable to the schema but a single lookup (available in NAACCR) can be used.
  • Version 9 Updated Diseases: AJCC released the 9th revision of the NET (excluding Adrenal Gland) and Vulva diseases which will be effective for cases diagnosed 1/1/2024 or later. There are now 2 schemas for each.

  • Version 9 Updates IDs: AJCC ID [#995] has been revised by the AJCC. This affects the AJCC IDs for Cervix Uteri V9, Appendix V9, Anus V9, Brain and Spinal Cord Other V9, and Brain and Spinal Cord Medulloblastoma V9.   • In Situ Calculation: For those schemas where AJCC does not define Tis but EOD Primary Tumor includes 000 for In situ, the Derived EOD 2018 T was being set to 88, but the Derived EOD 2018 N and Derived EOD 2018 M were being set based on the normal logic for the coded value. New calculation logic has been added to ensure that for these schema, if EOD Primary Tumor is set to 000, all of Derived EOD 2018 T, Derived EOD 2018 N, Derived EOD 2018 M and Derived EOD 2018 Stage Group are set to 88.   • EOD Primary Tumor [772]: Note 5 added describing how to code invasion of the muscularis propria; Codes 200, 250, 300 and 350 had ‘PATHOLOGICAL assessment only (requires a cystectomy)’ added to clarify when these can be used; Code 720 was modified to remove Colon, which is covered in code 650
• Kidney Parenchyma
  • EOD Primary Tumor [772]: Code 500 had ‘(excluding ipsilateral adrenal gland)’ removed; Code 600 moved ‘Adrenal gland (ipsilateral) (contiguous metastasis)’ from under ‘Extension beyond Gerota’s fascia’ to be separate section
  • EOD Mets [776]: Code 70 had ipsilateral Adrenal Gland separated from the Extension to list
• Lung
  • EOD Primary Tumor [772]: Note 6 was modified to clarify the coding of atelectasis; Code 675 was adjusted to remove Rib and add additional information for Adjacent rib
  • EOD Mets [776]: Code 50 had Adjacent rib (noncontiguous) added
• Merkel Cell Skin
  • Schema Selection: Histology ICD-O-3 [522] = 8041 with C51_ for 2024 and later was moved to Vulva V9

  • EOD Regional Nodes [774]: Codes 050, 100, 200, 300, 400, 500, 600, 700, 750 were removed as they do not apply for the 2024 Vulva N definitions; Codes 040, 110, 120, 150, 250, 310, 650 were added for the updated 2024 Vulva N definitions
