Notes

**Note 1:** pTX includes shave biopsies and regressed melanomas. **Note 2:** In addition to the depth of invasion, a Clark's level may be assigned by the pathologist. If there is a discrepancy between the Clark's level and the pathologic description of extent (invasion into the layers of the dermis), assign the higher T. **Note 3:** Code the greatest extent of invasion from any procedure performed on the lesion, whether it is described as a biopsy or an excision. **Note 4:** Satellite lesions/nodules or in-transit metastases are coded in regional lymph nodes. **Note 5:** Breslow's depth is coded in **Site-Specific Factor 1, Measured Thickness (Depth), Breslow Measurement.** **Note 6:** Ulceration of the melanoma is coded in **Site-Specific Factor 2, Ulceration.**
