**Note 1:** Physician statement of Primary Sclerosing Cholangitis (PSC) can be used to code this data item when no other information is available. **Note 2:** PSC is an idiopathic liver disease characterized by inflammation and fibrosis of the entire biliary tree. The chronic inflammation and injury to ducts may lead to cirrhosis and predispose to cholangiocarcinoma at any site in the biliary tree. **Note 3:** Code stated diagnosis of PSC either clinically or pathologically as documented in the medical record.
